A clinical incident reporting app for ADK Hospital, used by doctors, nurses and other staff.
Incidents are reported at the place they happened by scanning a QR code, which anchors the report to a location while it is still fresh. Reports can be submitted anonymously — that is the feature the whole thing turns on. Incident reporting only works if staff believe filing one carries no personal cost, and a system that quietly records who reported what will collect very few reports.
When a report is filed, the Quality and Safety Department, department heads and supervisors are notified by SMS. Notification by email would have meant a report sitting unread overnight.
Behind it is a form builder, because what needs recording changes: different incident types need different fields, and QSD refines them as they learn what they wish they had asked. Without that, every change to a form is a deployment.
